Agency for Integrated Care is a dedicated entity for an integrated healthcare system. Its primary role is to coordinate, manage and monitor referrals to government-funded nursing homes and facilities for the chronically ill. Manage patient transitions & coordination of all Intermediate and Long-Term Care services referrals. Its secondary role is to integrate the services of hospitals to those of nursing homes and other community providers for a comprehensive healthcare system, from hospitalisation to respite care.

Centre for Enabled Living is a first stop centre for users of eldercare and disability services, and their caregivers. CEL provides information and referral services, administers pan-disability schemes, provides support to caregivers and implements public education programmes.

Council for Third Age is an independent body that promotes active ageing to enable seniors to achieve a better quality of life in all the six dimensions of wellness – social, intellectual, physical, vocational, emotional and spiritual. They organise cultural, social, recreational, sports activities, as well as talks and other ad hoc events to encourage the older persons to lead active, healthy and meaningful lives. c) Senior Activity Centres
SACs provide a support network for the senior residents, especially the vulnerable ones. The services provided include maintaining a register of older persons, managing the alert alarm system, forming mutual help groups, providing information and referral services and promoting social interaction.

2. Home Care


Many older people prefer to live in a familiar environment with their family members and friends. However, in the event that the older person and/or the family cnnot manage to completely care or support the needs of the older person, home-based or community-based services may be required to help the older persons to continue staying at home, while providing some form of respite and support for their caregivers.

a) Befriending Services
This programme recruits and trains volunteers (including healthy seniors) to befriend and provide support to the isolated and vulnerable seniors who are homebound, have no family support and at risk due to physical health, social and/or psycho-emotional reasons. The volunteers visit these seniors on a regular basis and help them in simple household chores, running of errands and ad hoc escorts for medical appointments.

b) Day Care Centres (Social Day Care Centres)
These provide daytime supervision and care to frail seniors while their family members are at work or require respite from their caregiving roles. The programmes conducted by the Centres include maintenance exercise as well as social and recreational activities such as handicraft, singing and karaoke.

These provide a structured day care programme to accommodate older persons who suufer from dementia. i) Hospice Day and Home Care
Hospice is a specialised form of care designed to provide palliative care, physical and emotional comfort to patient and their families when a terminal illness no longer responds to treatments. The objective of hospice care is to prepare patients and ther families to attain a degree of mental and spiritual acceptance of death that is meaningful.

3. Community Care


Older persons should preferably be taken care of in their own homes or within the community for as long as possible. However, there are instances when caregiving cannot be reasonably provided or by their families.

a) Community Hospitals
This is an intermediate care facility between an acute hospital and community service, providing continuation of care from acute hospitals. Its objective is to provide functional independence of older persons, and caters to those who require long inpatient stay for rehabilitation nursing ad respite care after treatment in the acute hospitals.

b) Nursing Homes
The older person can be admitted into a Nursing Home if he/she needs daily nursing care and/or assistance in acitivites of daily living (such as toileting, walking, grooming and bathing) and if he/she has no caregiver at home. The older person must be dependent, wheelchair bound or bed bound, In addition, he/she must be medically assessed to be suffering from physical or mental dysfunction requiring a high level of nursing care.

c) Chronic Sick Facilities
These are specialised nursing homes that provide care to the older persons with complex medical conditions that require a greater level of care and supervision. In addition, their nursing needs such as tracheostomy, oxygen therapy and extensive bedsores, have to be managed by professional and trained nurses.

d) Hospice Inpatient Service
When the older persons’ condition becomes too difficult to care for at home, and the prognosis for him/her is less than 3 months, they can be admitted to an inpatient hospice. Inpatient hospice also provides short-term admission service for relief of distressing symptoms and/or during family crisis.

4. Caregiver Support


A caregiver is someone who has to take care of another person who is unable to live independently because of a mental or physical impairment. The major source of care comes from informal caregivers who often do not perceive themselves as caregivers but as wives, husbands, parents, daughters, sons or friends.

a) Caregiver Support Services
These services proide advice and support to caregivers through information and referral, consultations, training, support groups and other support programmes. They also conduct public education initiatives to raise awareness of caregiving in the community.

b) Community Case Management Services
CCMS is a service provided by professional case amnagers who are either social workers or nurses. Community Case Managers are trained to assess the needs of the older person and to find the appropriate services to support them and their caregivers.

c) Counselling for Seniors and their Families
Counselling services provide an avenue for older persons and their caregivers to voice their concerns and problems, thus improving their psychological and emotional well-being. The service includes face-to-face counselling, as well as information and referral assistance.

d) Elder Protection Service
This assesses and intervenes in cases of alleged domestic elder abuse abd us suitable for those suspected to be victims of elder abuse – be it physical, psychosocial, neglect, abandonment or financial.

TRANS SAFE Centre offers a range of services for people who are trying to manage issues of family violence, elder abuse and elder care. The Centre is also equipped with a video link-up system with the Family Court that allows victims of family violence to apply for a Protection Order.


5. Key Projects


ElderShield is an opt-out scheme for all Medisave account holders who are Singapre Citizens or Permanent Residents. Those who reach age 40 and who have Medisave accounts will be automaticallt covered under ElderShield if they do not opt out.

Interim Disability Assistance Programme for the Elderly (IDAPE) is a government assistance scheme to provide financial help to needy Singapore citizens who become disabled and need some cash to cope with expenses. IDAPE is for Singapore citizens who are not eligible for ElderShield because of their age or pre-existing disabilities.

The Primary Care Partnership Scheme was launched to help needy elderly Singaporeans to receive medical and dental services from private general practitioners at affordable prices.

SMRT Silver Tribute Fund is a corporate philanthropy and community outreach effort by SMRT, which raises funds for its beneficiary elderly VWOs.

Agelessonline is a free website for seniors and provides articles on health and wealth, opinions from other seniors, profiles of inspiring seniors as well as a list of seniors' discounts in Singapore. It is the hope of the founder/editor that it can break down stereotypes and be used as a resource to help seniors age well and be happy. Come read the article and follow their Facebook page. It also has an accompanying blog site called Agelessvoice that allows people in the ageing sector to share information.
